Medication Review

All patients who are on a regular prescribed medication will need a medication review annually (at least).

If you have been asked to amend your medication by a hospital specialist we would like you to make an appointment with a GP to discuss these changes. If you have a hand-written letter from the specialist this is particularly helpful to us as there is often a delay in receiving letters from the hospital.

Please be aware some medications require more frequent reviews and may include monitoring including blood tests. This is to ensure you are taking the correct medication, at the correct dosage for the correct duration.

In order to support this we have a PCN Pharmacist based at the practice called Asma Ghani. Asma attends the practice on a regular basis on Wednesday and works with the doctors and the patients to complete medication reviews. Please do not be surprised if Asma gives a call.
